Wegovy or Mounjaro?

Choose on two questions: how much weight loss you are aiming for, and what you can afford. Mounjaro led on weight loss in a direct trial. Wegovy costs £111 less on a first order at full dose.

1. Weight loss

SURMOUNT-5 is the only trial to put the two against each other. Over 72 weeks, 751 adults took the highest dose each person tolerated. Average loss was 20.2% of body weight on tirzepatide (Mounjaro) and 13.7% on semaglutide (Wegovy). Waists shrank by 18.4cm and 13.0cm.

2. How each one acts

Wegovy copies one gut hormone, GLP-1. Mounjaro copies two, GLP-1 and GIP. Both slow stomach emptying and reduce appetite.

3. Doses

Mounjaro has six doses up to 15mg. Wegovy has five standard doses up to 2.4mg, plus 7.2mg since January 2026. In its own trial, 7.2mg produced up to 20.7% weight loss. Nobody has tested 7.2mg against Mounjaro.

4. The contraceptive pill

Mounjaro's licence tells people on the pill to add a barrier method for four weeks after starting and after each increase. Wegovy's licence carries no such instruction.

A decision in four questions

  1. Is the highest possible weight loss your priority? The direct trial favours Mounjaro.
  2. Do you have heart disease? Wegovy has trial evidence of fewer heart attacks and strokes.
  3. Do you rely on the contraceptive pill? Mounjaro adds a four week barrier rule at each increase.
  4. What can you afford? Wegovy is cheaper at every dose today.

After stopping

Both medicines work only while you take them. An extension of the STEP 1 trial found people regained two thirds of the lost weight within a year of stopping semaglutide. SURMOUNT-4 found the same pattern for tirzepatide.
